Health Professionals / Medics

GPs, Consultants or health professionals can e-mail the details of patients that they wish to refer to CLEAR@brookes.ac.uk.
We then contact the patient to arrange a suitable start date.

Exercise Referral Criteria

The relevant forms should be completed by the referring Doctor (MD) or Health Professional (HP) confirming that

• the patient is clinically stable,
• the patient is compliant with medication
• the patient is not awaiting further cardiac medical or surgical treatment

 

Information required by the CLEAR includes

• Clinical event and date
• Subsequent complications or interventions
• Relevant medical history
• Medication

Forms

Before starting the programme of exercise patients will be required to complete questionnaires detailing their condition, medications and lifestyle.
